Among chronic pains, lower back pain is the most common: four out of five people will experience an episode. Chinese medicine gives it a particularly precise reading, because the lower back is not just any territory: it is the “home of the Kidney”, crossed by the Bladder channel, the longest in the body. When the lumbar region speaks, it is almost always the Kidney expressing itself. The question is not “is it the Kidney” but “which side of the Kidney”: and the answer reads itself in the reaction to heat.
The back: the territory of Kidney and Bladder
Two TCM structures make the lower back a special case.
The first is the Kidney, which “resides in the lumbar region”: its anchoring function, its substance (the Jing) and its heat (the Yang) are lodged in this zone. A background lower back pain that returns, that settles in over the years, is almost always read as a message from the Kidney: either its Yang no longer heats the region, or its Yin no longer nourishes it.
The second is the Bladder channel, which runs down alongside the spine in two parallel lines from head to foot: it is the great surface route of the back, the one that receives cold, wind and dampness coming from outside. A lower back pain triggered by a draught, an effort made in the cold or a night in a damp bed follows this path: the pathogen enters the channel, blocks it, and the lumbar Bi settles in.
Between the two: the hinge point of the neck and upper back (the “wind gate” points) is the entrance. This is why tradition recommends never leaving the back exposed to cold.
The cold lower back pain: Yang deficiency
First side, the most frequent: lower back pain of Kidney Yang deficiency. The fire that should heat the region declines; cold, internal or coming from outside, settles into the channel and contracts everything: the pain is cold, heavy, dull, the back is cold to the touch.
The signature is clear:
- dull, constant pain, worse in cold or damp weather;
- relieved by heat: the hot water bottle, the hot bath, lying down all relieve;
- accompanied by the general signs of Yang deficiency: chilliness, evening fatigue, night-time urination, weak knees;
- pain that “sinks” rather than burns, often across both sides of the lower back.
This is the lumbago of the “back that is cold”, the one that winter weather and insufficient rest keep alive.
The burning lower back pain: Yin deficiency
Second side, less known and yet typical: lower back pain of Kidney Yin deficiency. When Yin runs low, it is no longer cold that settles in but an empty heat that rises: the pain becomes burning, deep, in the bone.
The signature is the reverse:
- background pain with a sensation of heat or burning in the back;
- aggravated by heat and in the evening; the hot water bottle does not relieve, it can irritate;
- worse after fatigue and prolonged effort, relieved by rest;
- general signs of Yin deficiency: dry mouth in the evening, night sweats, hot palms, fine tinnitus, a complexion that flushes at times;
- pain often one-sided or centred, “in the bone” rather than in the muscle.
Distinguishing these two profiles is not a subtlety: they demand opposite prescriptions. Warming a Yin lumbago aggravates the empty fire; cooling a Yang lumbago drives the cold deeper.
Feeding the lumbar Kidney: the foods for the back
Dietetics follows the side, on a common base: support the Kidney without forcing.
For the Yang lumbago (cold pain, relieved by heat):
- warm, cooked meals, soups and stews, the morning congee;
- flavours that warm: ginger, cinnamon, cooked garlic and onion, leek, a little mild pepper;
- Kidney-Yang foods: walnuts, chestnuts, well-cooked lamb, sesame seeds, fennel, moderate natural salt;
- strict removal of dietary cold: nothing iced, excess raw food, cold drinks, especially in the evening;
- complementary gesture: the hot water bottle on the lumbar region in the evening, and the back kept covered.
For the Yin lumbago (burning pain, aggravated by heat):
- foods that nourish Yin: simmered soups, cooked pears, black sesame, tofu, eggs, fish, moderate warm dairy;
- regular hydration with warm drinks, never iced;
- avoid what consumes Yin: alcohol, excess coffee, chilli, grilled food, late meals;
- and above all night-time rest, which is the recharging of Yin: short nights are here a direct cause.
In both cases, the common rule: the Kidney loves regularity and hates exhaustion. It is not the intensity of a cure that rebuilds, but the constancy of the warm plate and the complete night.
What requires medical advice: the red flags
“Ordinary” lower back pain reads as terrain. But certain signs must send you to the doctor without delay:
- pain with fever, weight loss or general malaise;
- pain that wakes you at night and yields to no position;
- neurological signs: weakness of a leg, walking difficulties, numbness in the saddle area;
- associated urinary or continence troubles;
- recent trauma, or pain after a fall;
- history of cancer, or an elderly patient with a new pain.
The Yin/Yang reading orders the dietetics; it replaces neither the examination nor the imaging when the picture requires them.
