The plaque is red, sharply bordered, covered in silvery scales that fall like wax. It does not weep, it does not ooze, it flakes. Psoriasis is the opposite portrait of eczema, and this opposition is the whole of its TCM reading: where eczema oozes a damp terrain, psoriasis desquamates a dry one.
Psoriasis: skin that flakes without weeping
The skin needs Blood to nourish and moisten it. When Blood runs short, the surface dries and dies faster than it can be renewed: it forms scales. The underlying redness is the Heat that dryness no longer buffers; the sharp border is the lesion fixing itself where Blood can no longer reach.
This is the opposite mechanics of eczema, which weeps a Damp-Heat terrain. Psoriasis flakes a Blood Deficiency terrain with Dryness. Same word “inflammation” in the modern chart, opposite strategy on the plate.
Blood Deficiency generating internal Wind
The mechanics are precise. Blood Deficiency (blood insufficient in quantity or quality) dries the surface as a river running low dries its banks. The dried surface then generates internal Wind: the itch that moves, the scales that scatter, the flare that migrates from one site to another.
This explains two classic observations. The itch worsens at night (when Blood should return to the Liver and nourish the skin) and after scratching (which stirs the Wind further). And the lesion thickens: the skin, undernourished, accelerates its production in compensation, producing the characteristic plaque turnover that dermatology measures in days rather than weeks.
Nourishing Blood and moistening the surface
The dietary strategy is a feeding strategy, not a draining one. Two registers:
- Nourishing the Blood: the Blood-building foods, eggs, red meat in small cooked amounts, liver occasionally, beetroot, dark leafy greens, black sesame, goji, dates, cooked whole grains. The key is cooked and regular: the Spleen that transforms food into Blood needs warmth and rhythm.
- Moistening the surface: the Yin-moistening and skin-nourishing foods, pear, honey, sweet almond, sesame, tremella, lily bulb, fatty fish rich in omega-3. These are the gentle moisturisers that calm the internal Wind.
What to reduce: the drying and Heat-producing foods, alcohol (which dries and heats the Blood), chilli, fried food, excess coffee, smoking. And the heavy damp-producers, not because Dampness is the problem here, but because a weakened Spleen cannot transform them into the Blood that is needed.
Essential fatty acids: the double reading
Omega-3s deserve their own word. Modern dermatology knows they modulate inflammation and support the skin barrier; TCM reads them as foods that “nourish Blood and moisten Dryness”. Same food, two languages, one recommendation: sardine, mackerel, walnut, flax, rapeseed oil, in regular small amounts.
The caution is the Spleen. Fatty acids are dense; a Spleen already weakened (which is often the case in long psoriasis) transforms them poorly if they arrive in large raw quantities. Small regular portions of cooked oily fish serve better than supplements swallowed between two fast-food meals: the vehicle decides what the substance becomes.
Psoriasis is not eczema: why the opposite approach fails
This is the lesson this pair of articles teaches. The two dermatoses share a word (inflammation) and a surface (skin), but opposite terrains. Applying the eczema strategy to psoriasis, cold raw food, draining, low-fat, produces exactly what is observed: the plaque dries further, the scales thicken, the itch worsens.
Conversely, the psoriasis strategy (nourishing, moistening, cooked fats) applied to a weeping eczema flare feeds the swamp. The four-terrain reading of inflammation exists precisely for this: the diagnosis precedes the plate, never the reverse.
Frequently asked questions about psoriasis in TCM
Why does psoriasis flake rather than weep?
Because its terrain is dry, not damp. Blood Deficiency dries the surface, which produces scales; the Damp-Heat of eczema produces vesicles that weep. Same skin, opposite swamp.
Which foods help psoriasis?
Blood-nourishers (eggs, cooked meat, beetroot, dark greens, black sesame, goji) and moisturisers (oily fish, pear, honey, almond, tremella). Cooked, regular, and combined with fewer drying foods: alcohol, chilli, excess coffee.
Why does psoriasis itch at night?
Because Blood should return to nourish the skin during sleep, and Blood Deficiency leaves the surface unattended. The night itch is the signature of the deficiency terrain.
Can diet replace psoriasis treatment?
No. Modern treatments control the disease effectively; diet supports the terrain and can reduce flare frequency and severity. It is a complement, never a substitute.
The plaque asks to be fed, not drained
Psoriasis is the skin starving in plain sight: the scales are the surface dying faster than it can be nourished. The strategy feeds the Blood, moistens the dryness, calms the Wind, and lets the skin rebuild. Draining it further, in the name of “inflammation”, is treating the wrong disease.
